Description

Moduretic is a combination of a diuretic and antihypertensive drug where a potent natriuretic action of hydrochlorothiazide is combined with potassium-sparing effect of amiloride hydrochloride. Moduretic has diuretic and hypotensive effect, while the effect of amiloride is aimed at the prevention of excessive potassium loss that may occur to patients who take thiazide diuretics.

Active ingredients are amyl nitrite, hydrochlorothiazide

Pharmacodynamics

It is a combination drug which action is caused by components included in the composition; has a diuretic and hypotensive action. Hydrochlorothiazide – a diuretic average power of action, increases the excretion of Na+, Cl-, K+, Mg2+, bicarbonate; delays Ca2+ and uric acid in the body,. Amiloride decreases the excretion of K+ and Mg2+. The combination of components of the drug reduces the risk of development of hypokalemia and hypomagnesemia.

Indications for use

  • Essential [primary] hypertension
  • Secondary hypertension
  • Congestive heart failure
  • Fibrosis and cirrhosis of the liver
  • Ascites
  • Edema, not elsewhere classified

Method of application

It is taken orally, 1-2 tablets during a meal 1-2 times a day. Daily dose should not exceed 4 tablets (2 tablets 2 times a day).

Side effects

Headache, weakness, depression, sleep disturbance, hypotension, arrhythmia, diarrhea disorders, gout, joint pain, seizures, impaired renal function, hyperkalemia, hyponatremia, dehydration, impotence, rashes, etc.

Drug contraindications

Hyperkalemia (high blood potassium levels), renal dysfunction, hypersensitivity to the drug.

Pregnancy

Pregnant women should take this medicine only in those cases where the expected benefit exceeds the possible negative impact on the fetus.

Interaction with other drugs

ACE inhibitors increase the risk of hyperkalemia; alcohol, barbiturates, anti-hypertensives increase hypotension, NSAIDs reduce effects. Moduretic reduces the response to pressor amines, enhances the effect of non-depolarizing muscle relaxants. You should be careful with the dose in case of joint appointment with anti-diabetic drugs. It is not recommended to combine the drug with lithium, potassium and potassium-sparing medicines.

It should be prescribed with care to patients with liver failure, diabetes, with risk of developing respiratory or metabolic acidosis. In case of prolonged use it is necessary to monitor blood levels of potassium, sodium, chloride, urea, creatinine, glucose, and the level of uric acid.
